Dartmouth Health Community Health Team

The Dartmouth Health Community Health Team is a part of the Population Health Department and comprises of staff who focus on healthcare priorities outlined in the Community Health Needs Assessment. Some of the focuses include substance misuse prevention, food affordability, and socio-economic conditions affecting health and wellbeing.
